All clients must complete the Health History Form. Facial clients must complete both the Health History Form and the Facial Form.

Here's how you can receive 10% off your next visit and your referrals can receive 10% off their first appointment (excludes gift certificates). Refer your friends and family members and when they schedule and keep their appointment they'll receive 10% off their visit, and you'll receive 10% off your next visit.

Appointment Policies
- Please arrive 5 minutes early so we may start at your scheduled time.
- Late arrivals will still end at the scheduled time.
- Please call 24 hours in advance to reschedule or cancel your appointment.
- If you're sick the day of your appointment, please call and reschedule.
- No shows will be charged the full amount of the service scheduled. A payment for the now show appointment must be received by Body Spa & Massage, LLC before scheduling a new appointment.

Client Health History and personal information is collected and held in strict confidence. This information will never be shared outside my practice without the client's specific expressed written consent.
